Transitional Living Program

Youth Continuum’s Transitional Living Program is an apartment-style, 12-bed independent living program that can offer up to 21 months of stable house for youth ages 18-21 (at entry).  Youth who are unstably housed and/or are fleeing dangerous situations, and are not otherwise system or service connected are eligible to be referred for openings.  YC’s TLP has case management as a requirement and is structured as a life skills program that offers housing.

CHAP & CHEER Programs

The Community Housing Assistance Program (CHAP)/Community Housing Education and Employment Program (CHEER) programs provide scattered site, independent apartments, for young adults preparing for independent living. Youth receive weekly in-home Case Management services to learn life skills, complete their educational/vocational program, and develop the financial resources and community supports they will need for long-term success.

SKILLS Program

SKILLS Program (Stability, Knowledge, & Independence through Learning & Life Skills) provides intensive case management for 9 youth ages 18-21 in a supported, site-based apartment living setting helping to prepare youth for full independent living.

Rapid
Rehousing

Youth Continuum provides a range of supportive options for youth experiencing homelessness including up to 40 units of Rapid Rehousing for youth ages 18 to 24, funded by the federal Department of Housing and Urban Development. Eligible youth who are experiencing homelessness secure leases for apartments and are provided with a rental subsidiary, as well as intensive case management, job search and training supports, financial literacy and parenting skills while learning to live independently.  The youth’s continued need to receive the subsidy is assessed on an ongoing basis, as the youth and their case manager work in tandem to ensure long-term housing stability and prevent a return to homelessness.

Permanent Supportive Housing

Youth Continuum operates Connecticut’s first site-based permanent supportive housing program dedicated for chronically homeless and disabled youth in the State of CT.  These 7 dedicated units provide long term stability for these particularly vulnerable youth while also providing intensive clinic and case management services to help them become independent and exit the homeless service system.
